Severe Dandruff and Hair Loss

I have tried millions of things for both my dandruff and hair loss but still no positive effect. Oil treatments, regular shampoo, hair mayonnaise, even filtering water. I really don't understand my hair. Dandruff appears on the very same hour after shampooing. I thought I wasn't washing it off completely so I went to salon for getting my hair washed but as I came home, I felt the same thing.
